As part of the MassHealth Accountable Care Organization (ACO) program, CCBC has been designated a Behavioral Health Community Partner (BH CP) that partners with ACO in Brockton, Taunton, Attleboro and Southeastern Massachusetts to integrate and improve health outcomes of MassHealth members with complex long term medical and/or behavioral health needs. In its function as a Behavioral Health Community Partner, CCBC performs a primary role in meeting the physical, behavioral and long term care needs of members, including individuals with disabilities, mental illness, substance misuse disorders and co-occurring disorders.
Working collaboratively with CCBC’s Community Support Program (CSP), the BH CP is designed to build upon the successful engagement strategies of our CSP, in which the agency has achieved a wealth of experience working with Primary Care Physicians in Southeastern Massachusetts.

BH CP services include:
- Comprehensive Assessment and treatment planning
- Care coordination/management
- Medication Assistance
- Health and Wellness Coaching
- Connections to housing, transportation, and other supports
BH CP staff have strong working relationships with hospitals, Emergency Departments (EDs), inpatient medical-surgical units, and inpatient psychiatric units. Working as a Care Coordination Team, BH CP staff are skilled in engaging clients that have been referred from a variety of settings, and provide the continuity of care the individual needs to transition from inpatient medical to community settings. BH CP staff facilitate linkages to primary care from EDs, and timely access to outpatient behavioral health through CCBC’s Open Access model of care.
