Description of Position

CCBC is seeking an Intensive Care Coordinator (ICC) to join the Community Service Agency’s wraparound team for children aged birth through 21 with mental health needs. The ICC offers strengths-based, trauma-informed, and culturally competent care coordination. The ICC partners with youth’s treatment team and natural supports in coordinating and providing services such as facilitation of team meetings, collaborating with providers, linking to community-based services, and transition planning. The ICC is part of a multidisciplinary team and should have strengths in cultural humility, time management, building relationships and communication.

Duties and responsibilities of this position include:
  • Engage youth, caregivers, providers, and supports in establishing Care Planning Teams (CPTs) to address youth’s identified goals
  • Partner with youth, caregivers and team members in the completion of comprehensive assessments and treatment planning
  • Collaborate with youth and caregivers in promoting the involvement of natural and community-based supports in planning for each youth
  • Work collaboratively with providers such as educational systems, Department of Children and Families, clinical supports, and other stake holders to support youth in achieving their identified treatment goals
  • Provide services in the community
  • Complete documentation in line with agency and insurance requirements

Qualifications

  • Master’s degree in Social Work or related field, or
  • Bachelor’s degree in Social Work or related field and 3 years’ experience working within the child/family system
  • Must have a dependable car, valid driver’s license, and proof of insurance
  • Experience in navigating any of the child/family-serving systems and experience advocating for family members who are involved with behavioral health systems
  • Experience in Wraparound is preferred
