Description of Position

We are seeking a full-time Care Coordinator to join our CBHC team in providing assistance to our clients with mental health struggles. The ideal candidate is interested in providing a compassionate and full continuum of care to clients under the umbrella of the CBHC. You will be part of a multidisciplinary team of addressing the social, emotional, psychiatric, and medical needs of the clients we serve; this includes clients across the lifespan with a variety of presentations, including mental health and substance use concerns. Important qualities to be successful in this role include being self-motivated, good personal boundaries, and a belief in the principles of hope and recovery from mental illness. Responsibilities Include:

The Care Coordinator will provide support to the daily operations of the CBHC. These tasks will include:

  • Coordinating medical and behavioral health care and making referrals
  • Working to identify, mitigate, and lift social and health inequities and barriers and advocate for the individual's access to services.
  • Maintaining regular contact with client, provider, family, social service providers, and natural community supports to ensure treatment plans are implemented.
  • Providing education and developing safety plans with clients and families.

Requirements

  • Must hold a Bachelor's degree in a mental health field.
  • Must have at least three (3) years of human services related experience, preferably with individuals across the lifespan with serious mental illness and substance abuse.
  • Must possess good organizational skills and effective written, verbal, and computer skills.
