Description of Position

CCBC is seeking a dedicated, Masters-Level Clinician for our Behavioral Health Outreach for Aging Populations (BHOAP) program. BHOAP is a dynamic, multidisciplinary outreach team that works directly with older adults in the community. Our mission is to support their independence, enhance their overall quality of life, and provide comprehensive care addressing co-morbid medical and mental health needs. In this part-time role (20 hours per week), you will collaborate with a multidisciplinary team to conduct clinical assessments, deliver direct services, and manage documentation for elders looking to remain safely in their homes.

Key Responsibilities
  • Manage Referrals: Process incoming referrals for program intake and admission.
  • Conduct Assessments: Perform thorough clinical and psychosocial assessments.
  • Collaborate on Care: Develop and write individualized treatment plans in partnership with the elders.
  • Team Collaboration: Present assessments and clinical updates at multidisciplinary team meetings.
  • Community Outreach: Conduct regular in-home visits with elders out in the community.
  • Maintain Documentation: Complete all clinical documentation and incident reporting in a timely manner.

Qualifications

  • Education: Master’s degree in Social Work, Counseling, Psychology, or a related clinical field required.
  • Experience: Experience working with older adults or community-based outreach is highly valued.
  • Skills: Strong clinical assessment, documentation, and interpersonal communication skills.
