CCBC is hiring a Clinical Care Coordinator for its Behavioral health Outreach for Aging Populations (BHOAP) program. BHOAP is a multidisciplinary outreach team working with older adults in the community to support their independence and overall quality of life. The team works to assist older adults to remain in the community, live independently, and provides comprehensive support to address co-morbidities such as medical and mental health and substance use needs.

Description of Position

This position(s) will work as a part of a multidisciplinary team that conducts clinical assessment, provides direct services, and completes documentation for a group of older adults served by the BHOAP.

The Clinical Care Coordinator will focus on care coordination and aftercare needs to ensure older adults are connected to medical, mental health, and substance use services to support their ability to remain independent in the community. This position will provide in home services; facilitate outreach to local agencies to promote the services; and develop groups for underserved older adults as described below.

Responsibilities Include:
  1. Provide range of care coordination services to support older adults to connect or remain connected to services including medical, mental health, substance use appointments; housing and health insurance applications and any other care coordination needed to support the BHOAP client to remain safely in their home.
  2. Conduct outreach that will include meeting with local groups that address the needs of underserved older adults to increase referrals for BHOAP services.
  3. Development of a range of support groups that will occur weekly based on the needs of older adults to be served. Coordinate with clinicians and other BHOAP team members to develop the schedule of who will be leading the groups each week.
  4. Participate in bi-weekly staff meetings, staff supervision, and training.
  5. Complete documentation in a timely fashion, including incident reporting.

Requirements

This position requires a bachelor’s degree and experience or specific skills working with older adults in a care coordination role. Must have at least two years of human services with at least one year with older adults who have mental health and substance use needs; Must possess good organizational skills, effective written, verbal and computer skills. Experience in running groups preferred.
