Description of Position
The BH CP Supervisor ensures that Care Coordinators (CCs), Recovery Coaches (RCs) and Certified Peer Specialists (CPSs) are complying with BH CP Program requirements; provides training and support to CCs, RSNs and CPSs on Program requirements; acts as liaison to Primary Care Clinicians and behavioral health providers for high-risk or active members; reviews reports on assigned, engaged and discharged enrollees; convenes and facilitates daily organizational meetings; ensures delegation and completion of daily assignments; oversees daily/weekly staff responsibilities, staff meetings and individual/group supervision; ensures the enactment of each Enrollees Patient-Centered Treatment Plan (PCTP); coordinates services with EOHHS, ACO and other CP professional staff; triages crisis situations; enacts crisis interventions; completes documentation in a timely fashion, including incident reporting; and performs other duties as defined by agency and/or program leadership. Responsibilities
- Assist Care Coordinators, RSNs and CPSs in outreach, and engage assigned Enrollees using techniques of Motivational Interviewing; work collaboratively with client, family, behavioral health providers, and PCC's to complete Comprehensive Assessment (CA) and with Care Team to complete PCTP; follow up with Enrollee, provider, family, social service providers, and natural community supports to ensure PCTP is implemented; and actively participate in discharge planning meetings when Enrollee has presented/admitted into an Emergency Department or Inpatient setting.
- Advocating for the Enrollee's access to services.
- Facilitate sharing of CA and PCTP and updating as required with members of the Care Team using available technology and adhering to CCBC, State and Federal policies and regulations.
- Documenting activity in the CCBC Care Management Electronic Medical Record (EMR) Module.
Requirements
Minimum Bachelors degree, with work experience related to managed care and/or BHCP care coordination services, Masters preferred. The BH CP Supervisor will need to have two or more years of experience working in a Behavioral Health Community Partner, Community Support, Intensive Care Coordination or Case Management services setting. Program Supervision must have experience working collaboratively with state agencies, ACOs, consumer advocacy groups, and behavioral health outpatient facilities; and possess experience in navigating client-serving systems and experience advocating for identified clients who are involved with behavioral health systems.
Must possess good organizational skills, effective written, verbal and computer skills; ability to read clinical and population health reports; experience with clientele with Serious Mental Illness (SMI), Serious and Persistent Mental Illness (SPMI), Substance use disorder, and co-occurring mental health disorder, substance use disorder and/or chronic medical conditions. Knowledge of Core-Competencies as identified by the employee's assigned department; must have a valid driver's license in the state of residence and submit a copy of satisfactory record of driving from the state of residence of registry of motor vehicles, as determined by the department vice president; must complete MA and ADP Criminal Offense Record Information (CORI) process; and must have computer literacy as related to the requirements of the position.
