As part of the rollout of the MassHealth Accountable Care Organization (ACO) program, CCBC has been designated as a Behavioral Health Community Partner (BH CP) that partners with ACOs to integrate and improve health outcomes of MassHealth members with complex long term medical and/or behavioral health needs. In its function as a Community Partner, CCBC plays a primary role in meeting the physical, behavioral and long term care needs of members, including individuals with disabilities, mental illness, substance misuse disorders and co-occurring disorders.

Description of Position

The CP Lead Clinical Care Manager (LCCM) will directly onboard, train, manage, support and oversee the professional development of all CCMs, supervisors and direct staff across the department. LCCM’s will aid in addressing concerns and implementing disciplinary investigations to ensure that all programmatic guidelines, requirements and productivity standards are enforced across the program. LCCM’s will provide coverage in a supervisory capacity for leaves, absences and vacations. LCCM’s will additionally act as a liaison to Accountable Care Organizations (ACOs), DMH, Primary Care Clinicians, Emergency Departments, and inpatient facilities. The CMM will sign off on Comprehensive Assessments (CAs) and Patient-Centered Treatment Plans (PCTPs) as required by the MassHealth contract, convene and facilitate BH CP care team meetings, review reports, and help team to address gaps in care for Enrollees. LCCM’s will review and participate in the completion of updates to the BHCP Annual Reports and Request for Responses (RFR) for both BHCP and other related departments. LCCM’s will clinically support BHCP and interagency departments ie: BHJI, CSP/RC, Outpatient (TOD) pertaining to effective crisis intervention, de-escalation and resolution. LCCM’s will participate in planning, organizing, managing and supporting committee and agency wide initiatives ie: Wellness Event, CCBC Anniversaries, Staff Recognition, Food for Thought, Food Drives, Toy Drive and Agency Committees. Lastly, LCCM’s will sustain licensure requirements by completing necessary CEU’s.

Responsibilities
  1. Ensure that Care Coordinators (CC), Recovery Support Navigators (RSN), Certified Peer Specialists (CPS), Supervisors and CCMs and Registered Nurses (RN) are complying with BH CP Program requirements.
  2. Provide coverage in a supervisory capacity for leaves, absences and vacations.
  3. Ensure that all programmatic guidelines, requirements and productivity standards are enforced across the program, including incident reporting.
  4. Review and participate in the completion of updates to the BHCP Annual Reports and Request for Responses (RFR) for both BHCP and other related departments.
  5. Act as a liaison to Accountable Care Organizations (ACOs), DMH, Primary Care Clinicians, Emergency Departments, and inpatient facilities.
  6. Review and participate in the completion of updates to the BHCP Annual Reports and Request for Responses (RFR).
  7. Review reports on assigned, engaged and discharged Enrollees.
  8. Clinically support BHCP and interagency departments.
  9. Lead and oversee routine staff and programmatic meetings, along with group supervision.
  10. Ensure enactment of each Enrollees Comprehensive Assessment and Person-Centered Treatment Plan.
  11. Participate in planning, organizing, managing and supporting committee and agency wide initiatives.
  12. Sustain licensure requirements by completing necessary CEU’s.
  13. Other duties as defined by agency and/or program leadership.

Requirements

Master's degree, independent license required. Two or more years of supervisory experience required. Must have experience leading and facilitating teams that includes primary care clinicians, and hospital clinicians preferred, and participation on such teams required. The CCM should have experience working collaboratively with state agencies, consumer advocacy groups, and/or behavioral health outpatient facilities, as well as experience in navigating client-serving systems and advocating for identified clients who are involved with behavioral health systems. The CCM should possess good organizational skills, plus effective written, verbal and computer skills; have experience in Community Support, Intensive Care Coordination, Case Management, Clinical or Program Supervision; ability to read clinical and population health reports; and experience with clientele with Serious Mental Illness (SMI), Serious and Persistent Mental Illness (SPMI), Substance use disorder, and co-occurring mental health disorder, substance use disorder and/or chronic medical conditions.

Knowledge of Core-Competencies as identified by the employee's assigned department; must have a valid driver's license in the state of residence and submit a copy of satisfactory record of driving from the state of residence of registry of motor vehicles, as determined by the department vice president; must complete MA and ADP Criminal Offense Record Information (CORI) process; and must have computer literacy as related to the requirements of the position.
