The Partial Hospitalization Program (PHP) continuum of services offer a supportive and caring environment as a stabilizing base for clients to learn new coping and problem solving skills. PHP Elder Track
The agency's PHP Elder Track specifically focuses on the needs and challenges of the geriatric population, paying particular attention to complication factors that frequently arise with the use of multiple medications and co-occurring medical conditions.
Each PHP is operated by an experienced team of mental health professionals, including nurses, social workers, expressive therapists, licensed mental health counselors, and psychiatrists. The team also works closely with primary care physicians and home health nursing providers. The PHP provides psychiatric evaluation, consultation, and medication services on a case-by-case basis. Beginning with each client's admission process, the partial hospitalization team closely monitors the individual's mental status and designs an individualized treatment plan.
