Description of Position

Full time bachelor's level position providing a range of addiction and general rehab services to adults with mental illness. Program of Assertive Community Treatment (PACT) is a comprehensive, community-based psychiatric program providing a range of treatment, rehabilitation, and support services to adults with mental illness, many with co-occurring addictions.

Responsibilities

  • Completes assessments of client’s substance abuse history and current status as part of comprehensive assessment & on an ongoing basis
  • Engagement to develop trusting relationships to enhance successful substance abuse outcomes educational, behavioral, and motivational interventions
  • Assists in coordinating individual treatment planning including aftercare and recovery support services for each client actively involved in alcohol and drug treatment
  • Participates in engaging and motivating team clients in all of the treatment goals of the client, especially and specifically those related to substance abuse issues
  • Assists each client receiving drug and alcohol treatment in becoming involved with self-help support groups
  • Assists each client receiving drug and alcohol treatment in developing and maintaining social support networks
  • Assesses client’s need for inpatient substance abuse treatment and, when appropriate, coordinates access to these services
  • Collaborates closely with any inpatient substance abuse treatment providers
  • Ensures that each client referred by the PACT team for alcohol and other drug treatment is referred to an individual or program licensed or certified to provide alcohol and other drug treatment
  • Provide training to other PACT team members on the signs, symptoms and early identification of alcohol and other drug use and abuse, and the disease of alcoholism and drug dependency
  • Participate in the ongoing assessment and therapeutic engagement of PACT team clients
  • Transports and/or accompanies clients to appointments which connect them to community resources and services
  • Participate in PACT team meetings in order to facilitate an integrated, multidisciplinary team approach to service delivery

Requirements

A Bachelor’s degree in social work, counseling or a related field is required along with 2 years experience in addictions treatment Have documented competency in the following areas, or receive core training to be completed within six months of hire: Engagement strategies, Motivational strategies, and Relapse prevention.

Must have a valid driver’s license in the state of residence and submit a copy of satisfactory record of driving from the state of residence of registry of motor vehicles; must complete MA and ADP Criminal Offense Record Information (CORI) process; must have computer literacy as related to the requirements of the position.
